PatientCareLinks is a digital health platform created by the Preventive Health Institute (PHI) to help people take a more active role in their health — and to help doctors provide better, more continuous care.
We believe healthcare works better when patients are not simply treated when they become sick, but are supported to understand their health, make better decisions, and stay connected to their healthcare over time.
We also believe that doctors need better tools to organise, follow up, and share knowledge — especially those caring for private patients outside formal hospital settings.
That is what PatientCareLinks is about. It is a tool for both patients and doctors.
PatientCareLinks brings together a growing collection of practical health resources in one place — and gives doctors a digital home for the patients they already care for.
And we are continually developing new tools for both patients and doctors.
Traditional healthcare often captures a moment.
You become sick.
You visit a healthcare facility.
You are examined.
You receive treatment.
You go home.
But your health does not exist in isolated moments.
Your weight changes. Your diet changes. Your blood pressure changes. Your medications change. Your activity changes. Your symptoms change. Your laboratory results change.
The real story of your health is the pattern that develops over time.
PatientCareLinks is developed around this idea — for patients and for the doctors who care for them.
We want to help create a more continuous relationship between you, your health information and your healthcare team.
Our goal is not to replace doctors or hospitals.
It is to make healthcare more useful before, during and after the consultation.
We believe that better health comes from understanding the things that influence it — nutrition, lifestyle, behaviour, medications, medical conditions, family history, measurements and the changes that occur over time.
The earlier important patterns are recognized, the more opportunities there may be to do something about them.
That is why PatientCareLinks places such importance on education, prevention, monitoring and continuity of care.
PatientCareLinks is part of the Preventive Health Institute, a healthcare organization.
We are not simply building a software product and asking healthcare providers to use it.
We are building PatientCareLinks from our experience of actually providing healthcare.
That gives us a different perspective.
We see the problems patients face.
We see the problems healthcare professionals face.
And we see how much information is lost when the patient, the healthcare professional and the healthcare system are disconnected.
PatientCareLinks is our attempt to build a better connection — for patients, for doctors, and for the health system as a whole.
PatientCareLinks is a place to share and create health resources — for patients, for doctors, and for the wider community.
Doctors using the platform can contribute appropriate educational material from their clinical experience:
Patients gain access to a growing library of trusted health resources, and doctors help build a body of knowledge that benefits everyone.
Patient confidentiality matters. Any information is encrypted and handled in accordance with applicable professional, privacy and consent requirements.
Technology is not the goal.
The goal is better healthcare.
We use technology because it can help patients access information, communicate with healthcare professionals, keep useful records, monitor changes and remain engaged with their health.
It can also help doctors organise their practice, follow up with patients, and share knowledge — without losing the human relationship at the centre of care.
We also believe that artificial intelligence will increasingly become part of healthcare.
But AI should support healthcare professionals and patients — not replace the human relationship at the centre of healthcare.
We do not believe we have already built the perfect health platform.
PatientCareLinks is an evolving project.
We are continually adding tools, learning from patients and healthcare professionals, and asking a simple question:
“What could make healthcare more useful for this person?”
Some of the things you see today will become better. New services will appear. Other ideas may change or disappear.
What will remain is our underlying purpose:
Whether you are a patient looking for a doctor, trying to understand something about your health, looking for healthier choices, or monitoring a health condition — or a doctor wanting to organise your private practice and give your patients a digital home — we want PatientCareLinks to be a place you can return to.
Healthcare is not just what happens when you are sick.
Your health is a journey.
PatientCareLinks is here to help you navigate it.
